How to Add or Change Plan of Care (POC) Text for SOAP Notes?

The Plan of Care (POC) declaration note in SOAP notes is crucial for ensuring legal and regulatory compliance, promoting transparency, and simplifying processes. It affirms adherence to standards and commitments, helps in initial verification, and reduces administrative burdens.

To update or add POC text in your SOAP notes, follow these steps:

Step 1: Start by logging into your Apollo dashboard using your credentials.

Step 2: From the dashboard, click on "Settings”.

Step 3: In the settings menu, select "Manage Templates”.

Step 4: Click on "Other Templates," then find and select "Edit POC Custom Text”.


Step 5: The editable text page will appear. Here, you can add or modify the POC text according to your business requirements.




Step 6: Once you are satisfied with your edits, click on "Save Template" to apply the updated POC text.


Step 7: To view your changes, go to the patient SOAP notes and select "Initial Examination Note”.


Step 8: With the Initial Examination Note open, mark the "POC Approval" checkbox.

Step 9: Fill in any other required information and click "Save & Continue.”



Step 10: Click the "Print" button. Scroll to the bottom of the printed document to verify that the updated POC text appears as intended.
