How to create an addendum of a completed note?

Description:
An addendum allows users to make additional notes or corrections to a completed SOAP note. It is important to understand that the addendum feature is only available once the original note has been marked as Completed—it is not visible or usable for notes in Incomplete status.

  • The original note cannot be edited once completed; only addendums can be added.

  • Users can create multiple addendums if needed, and each one will be listed chronologically.

Steps to Follow

Step 1: Log in to the Application
Start by securely logging into the system using your registered user credentials.

Step 2: Go to the Patient Section
From the dashboard or main menu, click on the "Patient" section. This will take you to the list of registered patients.

Step 3: Open Manage Patient SOAP Notes
Select the relevant patient, then navigate to the “Manage Patient SOAP Notes” option. This area displays a complete list of SOAP notes associated with the selected patient

Step 4: Add SOAP Note
User clicks the 'Add SOAP Note' button, they should be navigated to a popup displaying a list of existing notes.

Step 5: View the List of SOAP Notes
Here you will see all SOAP notes, along with their statuses (Completed or Incomplete).
Look for the note you wish to add an addendum to—ensure its status is Completed

Step 6: Click on the “Addendum” Button
Next to each Completed SOAP note, you'll see an “Addendum” button. Click this button to begin adding supplementary information

Step 7: Write and Save the Addendum
You will now be redirected to a dedicated Addendum/SOAP Note screen. Enter the necessary updates, comments, and corrections in all the provided fields.

 

Step 8: Finalize the Note — Addendum Successfully Created
